Custom Cooked Questionnaire - Pages Format

Apple Pages Format Downloadable Questionnaire (please see other listing if you need Microsoft Word format) - Answer a list of questions to help me get to know your dog's needs, and I'll put together a custom recipe just for you. In the questions I discuss allergies and picky eaters, but there's space for you to provide more details if I didn't ask a question that you have concerns about. Download the file, fill out the form, save it, and email it back to me. Then I will put together a recipe based on your answers. Allow 3-5 days turnaround time to receive your finished recipe, especially in cases where I may need more info based on your answers.
 
By purchasing this product, you understand and agree that your custom recipe may be listed for sale at the regular recipe price after it is created. The price you pay now includes the cost of the recipe and a service charge for putting the recipe together. 
 
When your purchase is complete, please wait for the confirmation page to load where you'll see the download link. You will also receive an email confirmation with a link to download that is good for 30 days.
